Description of resource: Advanced Illness Management and Hospice Programs Dealing with a serious illness is not something you need to do alone. The Advanced Illness Management Program and Hospice Program at Community Nurse Home Care provide practical and specialized help intended to comfort and support families and individuals at any stage of a life-threatening illness. A sense of security is instilled knowing that professional help is readily available. Our highly trained clinicians will assist with symptom and pain management. Patients of the Bridge program have full access to the wide variety of home care services available at Community Nurse Home Care.

When the decision is made to transition to Hospice Care our patients and their families can feel secure in knowing that the members of their team will not change. Their  nurse and home health aid are joined by new members of the paitents care team. Patients who are on our Hospice Care program have a full interdisciplinary team supporting their specialized needs Our Hospice Interdisciplinary Team includes:
