Description of resource: LifePath, formerly Franklin County Home Care Corporation is the local Area Agency on Aging and Aging Services Access Point serving Franklin County and the North Quabbin Region. Our mission is to help older Americans, people with disabilities, and caregivers maximize their independence and quality of life through agency programs, referral, and coordination with area providers. We help elders and persons with disabilities maintain independence and quality of life in their own homes and communities. We help busy caregivers to find relief and help loved ones to choose the right path.
