Description of resource: Spectrum Home Health & Hospice Care provides quality care that comes to you, in the privacy and comfort of your own home. We coordinate medical care with community physicians, families and multiple service agencies. Services, including wound care, medication education, medication monitoring, and therapy, are delivered throughout Hampden County, serving homes in Longmeadow, East Longmeadow, Springfield, West Springfield, Agawam, Chicopee, Hampden, Ludlow, Westfield and Wilbraham. Patients are referred to our home health and hospice services by acute care hospitals, physician offices, and skilled nursing facilities, as well as personal choices and recommendations.
