East Boston Neighborhood Health Center

Description of resource: The Elder Service Plan of the East Boston Neighborhood Health Center (ESP) is a community-based program that offers a complete range of health and health-related services designed to keep frail elders living in the community for as long as possible. Coordinated care is planned and provided by a multidisciplinary team of providers that includes physician, nurse practitioner, nurses, social workers, rehabilitation and recreation therapists, health aides and others. Working in partnership with each participant and their caregivers, the teams create individually designed care plans to best meet the needs of each person enrolled in ESP.
