VNA Care Hospice

Description of resource: VNA Care Hospice, an affiliate of VNA Care Network, was the Commonwealth’s first Medicare-certified hospice. The nonprofit agency provides a continuum of care for individuals and families facing a terminal illness in eastern and central Massachusetts. The interdisciplinary hospice team offers a combination of physical, emotional, practical, and spiritual support to help hospice patients and their families live as fully as possible. Hospice is provided in patients’ homes, three home-like hospice residences, and assisted living, residential care, community support and extended care facilities. Comprehensive bereavement support is available as well as free bereavement support groups that are open to the public. Care Choices services, offered in conjunction with the nonprofit home health care agency VNA Care Network, are available for patients with a life-limiting illness who are seeking curative/restorative treatment or who are not otherwise candidates for hospice. Care focuses on pain and symptom management through consultation and intervention provided by hospice staff. If a patient becomes hospice appropriate, a seamless transition can be made from one program to the other. VNA Care Hospice is licensed by Medicare and Medicaid and accredited by JCAHO.
