Description of resource: The Massachusetts General Hospital (MGH) Palliative Care Service is a medical support program that provides comprehensive care to patients and their families facing a life-threatening or terminal illness. The focus of care is to support the medical, psychosocial, and spiritual needs of the patient and family. A hospital-based team includes physicians, a nurse practitioner, and clinical social worker, a part-time chaplain, and a bereavement and volunteer coordinator. The Palliative Care Team sees patients in a variety of settings, such as inpatient, outpatient, and home to provide pain and symptom management, support for end-of-life decisions and planning, counseling, post hospital care planning, and bereavement follow-up.
