Harvard Vanguard - Altrius Health Palliative Care Program

Description of resource: The Harvard Vanguard Medical Associates, Palliative Care Program provides care for patients who are Harvard Vanguard members and are in the late stages of ANY life-limiting illness. Our interdisciplinary team works with you and your other care providers to address such issues as alleviation of physical pain and other symptoms, opening up communication between patients and their care providers and within families, assisting with decisions around artificial nutrition and hydration, and assisting with psychosocial and financial issues. We see patients at the Brigham and Women’s, Faulkner, and Mount Auburn Hospitals as well as at HVMA practice sites, at long-term care facilities, and at patient’s homes. Our goal is to provide compassionate, comfort-focused care.
